Some of this info I got from my friends. So give them credit too!
My friends Lisa and Alejandra are real helps when it comes to pokemon, so go to their blog to show your apprieciation!
Just click on either of these links luvlipups.blogspot.com or mytop10anime.blogspot.com and post a comment! I have a post about these blogs, so you should read how good they are!

Or just click on this link, and you will get the pokemon you're looking for. (kinda) This is only for the Sinnoh pokedex. http://pokemondb.net/evolution I think the link below has all evolutions.

  1. To get 5 stars on your trainer card you'll need

Well, you have to complete all the task below. (In Diamond/Pearl)
    -Beat the Elite Four.

-Get a ribbon from any Master Rank Super contest.
-Capture all Pokemon, not including Mew, Lugia, Ho-Oh, Celebi, Jirachi, Deoxys, Phione, Manaphy, Darkrai, Shaymin and Arceus (credit to Bulbapedia).
-Get a 100-chain victory at the Battle tower.

-Capture at least 50 flags in the Sinnoh Underground. Yes the flags can be from the same person

It's really hard to complete it. Try your best.
An added bonus is that if you get at least four stars in your Trainer Card, the nurse at any Pokemon Centre will remember you and treat you like a VIP everytime you request for healing. Not ground-shaking, but hey, its something special. My but. but dont use action replay.
And Yes the flags can be obtained from the same person

    -have gotten the national dex
    1. fly to twinleaf with a team of one fire pokemon and four eggs
    2. save and turn the game off
    3. turn the game on and walk to lake vierity
    4.go back to route 201
5.catch a bidoof and raise it to lv.23 [dont evolve it or go off the route or else the cheat will be reset]
6.fly to sandgem town and go to the professor [if you done the cheat right, he wont be there]
7.fly to twinlwaf and he will be waiting there. he will give you all the event pokemon items including heavens flute!
if he doesnt appear then he should appear the second time you do the cheat

  3. I got this info from wikianswers... It's either to get 1 or 2 stars on your trainer card:

There are five key accomplishments that will level up your trainer card. In no particular order:

    * Beat the Elite Four and enter the Hall of Fame.
    * Complete the National Pokedex.
    * Earn the Platinum Flag in the Underground.
    * Win a Master Class Super Contest.
    * Get 100 consecutive wins at the Battle Tower.

For each one of these you complete, a star will be added to your card and your card will change color.
I belive it doesn't do anything, just shows your big accomplishments, and gets you respect from other players in the uinion room
